NARR Forms - Multiple Transfer

Use this form to record a change of ownership for multiple rats (up to 4 on one form) from one person to another. This form is typically used to transfer ownership from a breeder to the new owner for rats registered as part of a Group Litter Member Registration.

On the form, mark the appropriate membership type that applies to you.
Enter the Previous Owner's Information in the space provided, and have the previous owner sign the form.
Enter your information in the spaces provided for "New Owner's Information."
Enter the rat's registration numbers and registered names in the spaces provided for "Rat Information."
Supply any updated health information you have relating to these rats, and note which rat you are referring to.
Print, sign, date, and mail the application with the appropriate fees (if applicable), or fax the form to the number listed on the member's only page of the main site.

Important! For rats not owned by the original breeder, this form MUST be signed by BOTH the previous owner and new owner to be accepted!
